Once your spot in your selected class is verified, Confirmation of Enrollment will be sent to the email address with which you have provided us.
Soccer Stars United Enrollment ONLY: Soccer Stars United class enrollment is subject to Approval and availability. For Players who are not SSU Approved, Enrollment is subject to Player Evaluation in the selected Soccer Stars United class. Players will be added to the class for the remainder of the season following a successful Player Evaluation. In the event of an unsuccessful Player Evaluation, administrators will assist with identifying an appropriate Super Soccer Stars class option. If your player is already SSU Approved your Soccer Stars United Enrollment will be confirmed subject to availability in the class.
Enrollment Agreement, Payment, Refund and Make-up conditions
I am the parent/guardian
and I certify that my child has no known medical or other conditions that could interfere with his/her participation in Super Soccer Stars’ activities. I also understand and accept that the practice of soccer, both indoor and outdoor, involves certain risks of physical injury. Therefore, I, individually and on behalf of my child listed above, hereby release, discharge, and hold harmless Round Star, Inc. d/b/a Super Soccer Stars and each of its respective parent, subsidiary, and affiliated companies, and each of their respective officers, directors, agents, representatives, employees, successors, assignees, and licensees (hereinafter, "the Released Parties") from any and all claims, actions, damages, losses, liabilities, costs and expenses of any kind whatsoever, including but not limited to any claims of negligence, arising out of, resulting from, by reason of, or in connection with my child's participation in any Super Soccer Stars program or activity. I agree that I will not bring or be a party to any legal action or claim against the Released Parties, or any of them, based upon or arising out of my child's participation in any Super Soccer Stars program or activity on any legal theory whatsoever (including without limitation personal injury, negligence, rights of privacy and publicity, or defamation).
I understand and agree that Super Soccer Stars has the right to terminate the enrollment of any child from Super Soccer Stars if it, in its sole discretion, determines that the continued attendance of the child in the program is not in the best interest of the child or the program. I agree to supervise or designate a responsible adult to supervise my child prior to and immediately after his/her Super Soccer Stars activities. I understand and agree that in case of an allergic reaction, I am solely responsible for attending to and treating my child. Super Soccer Stars coaches are not authorized to administer medication or EpiPens.
Super Soccer Stars and its agents reserve the right to take and utilize photographs, videos, or any type of recordings of participating children and their parents, caregivers, or anyone accompanying them to Super Soccer Stars while engaged in our classes or associated activities. I consent to Super Soccer Stars' use of the photographs, videos, or recordings of my child, me, or the guardian I have designated for advertising, promotional, or related purposes, and waive all rights to compensation and other rights which may arise as a result (including any rights under N.Y. Civil Rights Law 50). I understand and agree that the email address(es) I have provided herein will be added to the distribution list for Super Soccer Stars and its affiliate programs.
Payment, Refund, and General Enrollment Conditions:

Payment for Soccer Stars United Travel Program will made through reoccurring credit card billing, 2 installments of $950 in July and January and one installment of $755 in March. The first payment will be billed upon completion of this application, and the additional two of the 3rd of January and the 3rd of March. Credit Cards which are set to expire must be updated before the next billing date. The cost of the program will cover coaching expenses, rental fees, administrative expenses, league fees and applicable tournament fees. The uniform fee of $175 will be paid to Upper 90 through the emailed ordering form. The uniform package includes: 1 white Jersey, 1 orange jersey, 1 pair of black shorts, 1 pair of white socks, 1 pair of black socks, 1 Soccer Stars United backpack, 1 pair of warm up pants and jacket. Super Soccer Stars reserves the right to cancel practices, games and tournaments due to inclement weather. No make-ups or refunds will be given for missed or canceled practices, games or tournaments. Rescheduling of canceled CJSL games is contingent to the availability of fields and is not guaranteed by Super Soccer Stars. During the fall and spring seasons, travel teams will practice 2 times per week. The majority of CJSL games will be scheduled on Saturdays, although occasional CJSL games may be scheduled for Sundays. Winter Season will include 1 practice per week, 1 practice on Saturdays, Chelsea Piers youth indoor soccer league games and periodic winter tournaments on weekends. Practices, games, and tournaments schedules are subject to change. In the event that you decide to withdraw your child from the Soccer Stars United Travel Program, you must notify the Super Soccer Stars office prior to the month of your next bill date. You must give at least one month notice in the event you decide to withdraw before the start of a season. You are responsible for paying the monthly fee during any month in which your child participates in a Soccer Stars United Travel activity. Children who wear glasses must use shatterproof eyewear at all times. Children MUST wear appropriate footwear: turf or molded cleats for outdoor games and indoor soccer shoes for indoor practices and games. Shinguards must be worn to all practices and games. Super Soccer Stars is not responsible for your personal belongings; please keep your items with you at all times. Your purchased uniform must be worn to every game. Soccer balls are also available ($15 each).
